All areas in GREEN represent mandatory information. A printing request will not be
processed without this information.
Always include a sample of the project you want reproduced. This will help us to understand what you have asked
us to reproduce.
Date - Insert date the printing request form was filled out.
Dept/School - Insert your department or school name. If this name is different
than the account number, than insert the department or school that this project is shipped to. This box indicates
to us where the shipment goes.
Job Name - You must give the job a name. Pick any name that helps you remember what
the job is. When checking with us on status, we need the job name in order to find the project quickly.
Number of copies needed - Insert the total number of copies you would like us
to produce.
Job Number - Do not put anything in this box. This is the number printing services
assigns to this job while it goes through our shop.
Contact name & phone number -Insert the name and phone extension of
the person who knows the most about this job. If we have any questions we need to talk to this person.
Job form number - If you have assigned a form number to your printing job please
insert it here. When we reprint a job in the future, we can find it much faster if we look for a form number
rather than a description.
Number of originals supplied - Insert the number of originals you are
supplying us. If the number of originals we receive does not match this number, we will put the job on hold to
make sure that originals are not missing. The number of originals should be the number of sheets you send us, not
pages.
Print One Side, Print Two Sides - Check the box that indicates how
you want your final product to look; one sided copies or two sided copies. If you have both one sided and two
sided copies in your document, click both boxes. Please note, the computer will bill the entire document as
either one sided or two sided. We can print a combo of one sided and two sided, but you will be billed as though
the entire document is two sided, making it more expensive.
Check here if there are changes from last run
***IMPORTANT***
You must check this box if you have made changes from the last time we printed this job. We receive so much work
that we do not have time to check every original for marked changes. If this box is checked then we know to check
the document for changes. If this box is not checked, then we will automatically re-print the last
version. If we print the wrong version because you did not check this box then you will be charged for a
reprint.
Paper Color - This line should indicate the color of either the cover or text of your
document.
Cover - This stock is thicker than text stock. It usually is used as the cover to a book or as an index
card for school registration cards.
Text - This stock is the thickness of a regular piece of Xerox paper. It is usually used for the inside
pages of a book or as regular handouts. If you are submitting an NCR (carbonless form), please indicate the
number of copies per NCR form ( 2 part, 3 part, 4 part, etc.)
Ink Color - This line should indicate the ink color of either the cover or text of your
document.
Cover - This stock is thicker than text stock. It usually is used as the cover to a book or as an index
card for school registration cards.
Text - This stock is the thickness of a regular piece of Xerox paper. It is usually used for the inside
pages of a book or as regular handouts.
Finished Size - This indicates the finished size of your project.
Folded, Perfed, Collated, Tape Bind - This line tells us how
you want us to bind your document. You should include a sample of your project to help us understand what you
would like done.
Folded - Check this box if your project folds.
Perfed - Check this box if you want perforations on your project.
Collated - Check this box if you want the copies we make collated in the same order as your originals. If
you send us collated originals, but you do not want the copies collated as the originals are, than do not check this
box.
Tape Bind - If you are having a book reproduced and you want it tape bound than check this box.
Stapled - This line indicates how you would like your multiple copies finished.
One in left corner - A staple in the top left corner to hold multiple pages together.
Saddle stitch - This is two staples in the spine to create a bind such as a magazine or booklet. Please
note that this type of bind must have a number of pages that is divisible by four since the pages are folded, collated
together and then stapled.
Perfect bind - This is where the cover of a book is glued to the text pages. Examples of this are the
workbooks the district produces, such as the McMillan practice, spelling and grammar books.
Punched - If you would like to have your finished project drilled, please check the
appropriate box for the number and size of holes you would like. Include a sample of the project to help us
understand the side you would like drilled.
Number from - If you would like the individual pieces to have ascending number printed
on each, then indicate the starting number and ending number. An example of this would be your checking
accounts. They start with a number and go in order till the last printed piece. Make sure you keep track of
the last number, so when you have the job reprinted you will know with which number to start.
Number of Pads, Number of sheets per pad - If you would
like to make pads out of your printed piece, such as a telephone message pad, than indicate how many pads you would
like and how many pages in each pad. If you choose this option than all you need to put under "copies
needed" above is the number of pads you would like.
Date needed ***IMPORTANT*** - You must put a realistic
delivery date in this space. A rule of thumb is to allow at least a week for us to reproduce your
piece. Often we can deliver in several days and will deliver the project as soon as it is completed. If you
need it sooner, please call and let us know.
Do Not use ASAP as a delivery date
24-hour rush - As it says on the printing request form immediately under the 24 hour
rush check box, there are restrictions to this service. If your project does not meet these restrictions, then
please call us and discuss the project and a delivery date with us.
Authorized Signature - The person authorized to sign a printing request slip
must sign in this space before it is sent to us. A signature indicates that we are authorized to produce this
project and that there are sufficient funds in the account listed to pay for this project. If you would like to

our prices listed. If your project is complicated and you cannot figure a rough price from the pricing page
please call us.
Account Number - A valid account number must be listed. There must be enough
money in this account to pay for the project. If you would like to estimate the cost of this project, please

White, Pink, Hard Copy
White copy - You must keep this copy for your records. It is very important that you have this white copy
in front of you when calling to check on the status of an order. There is certain information on this copy we
will need to find the job.
Pink copy - This copy must be sent to us along with the hard copy when you send in your order. This copy
will be adhered to one of the boxes when the job is delivered to you. You must keep this copy in your records
when it is returned because it will have the job number assigned by us on it. If you have any questions in the
future about the billing on this job we will need the job number from this copy to find the job.
Hard copy - This copy must be sent to us along with the pink copy when you send in your order. This copy
will remain in printing services for our records.
