Reimbursement

Use this form to request expense reimbursement for test prep classes, tests, or classes listed in your Title II Professional Development Plan.  This form must be completed prior to participation.  Remember registration is the only item reimbursed.  After completion of the class, test prep, or test, submit original receipts, test scores, and verification of attendance, attached to a copy of the previously submitted SAUSD Conference Form and return to:  Diana Diaz, District Office.

Please remember to keep duplicate copies for your files.  Reimbursement is for SAUSD certificated employees only.
